Sometimes (it seems to happen for sites I visit a lot like daringfireball.net or twitter.com, but that's probably just frequency) when I type in a website address, Firefox will ask me to "Switch to Tab" but I don't have a tab open for that site anymore.

(FWIW I've observed myself to be atypical amongst other heavy browser users: I don't have very many tabs open at once, unless I'm actively doing research or something -- the upshot of that being that I open and close a lot of tabs.)

I'm using Firefox 4.0b7, but this has been happening definitely in b6, perhaps before then.
The worst part of this bug is that:

the "Switch to tab" entry in the awesomebar doesn't actually do anything, making that exact url inaccessible. Usually you can get away with deleting a / or munging the URL to go to a 404 page and then clicking back to the site's main page or whatever, but still, yeah that sucks.

we solved some of those bugs in b7, but looks like there is still some, we are missing steps to reproduce though.

Sorry for the bug spam, but you can work around this.  You can edit out the "switch to tab" piece in the location bar (eg. home and backspace) and then go to that page.  Not pretty but easier than typing the whole thing by hand.
