I am having this problem every few days. I start Thunderbird and my IMAP folders tree only shows Inbox, Sent and a new INBOX/SPAM folder. 

If I try to subscribe from Thunderbird, marking the preferred folders, nothing happens. 

I need to access my IMAP mailbox via webmail, I subscribe to all the folders and the next time I open Thunderbird I find all the folders with all their messages.

My ISP says the mail server only answers to the orders it receives from Thunderbird. 

Reproducible: Sometimes

Steps to Reproduce:
1. Open Thunderbird. 
2. Sometimes, every few days, folders have dissapeared.
3. Solution: Subscribe to folders via webmail.
Actual Results:  
Folders appear again for some days.

A secondary effect of this problem is that, after the folders appear, Thunderbird needs to download all the headers of the thousands of messages.

Are you saying that your ISP is losing your subscription information? Thunderbird won't unsubscribe you from your folders unless you ask it to. It rather sounds like they're losing subscription information. You could try turning off the use of IMAP subscription in Thunderbird (tools, account settings, server settings, click the advanced button, and uncheck the "only show subscribed folders button") and see if that helps. Or, you could try IMAP protocol logging and save off a log where this happens, especially when you try to resubscribe yourself to folders that you were subscribed to before but it fails - https://wiki.mozilla.org/MailNews:Logging
