For the scary billboard (Bug 615751) we point to a survey to ask why users don't upgrade to the latest Fx version. The survey was done in the past (see attached) but the metrics team does not know where that survey lives and is not sure if it was HTML. Anyway, can we get the attached survey in HTML so we can link to it from the billboard. Our goal is to launch the billboard around Dec 20th. Also, this survey should be localized if possible since the billboard will be localized.

Is this a surveygizmo survey? That seems to be how other surveys on mozilla.com are done.

I'll create the survey in SurveyGizmo and send the embeddable survey code
Which code do you need? Javascript, Static HTML, iFrame? I was going to add it to the bug but the static HTML is pretty long. If you can't get access to it in SurveyGizmo let me know and I can attach a word doc with the code. The survey question is now in Survey Gizmo and is called "Firefox Upgrade Survey".

The survey should be localized. Given I created the English one in Survey Gizmoa would that mean that I need to create those surveys in Survey Gizmo for each language?

Laura Mesa knows how to use surveygizmo (I don't). Comment #0 says that the survey should be localized but nobody from our l10n-drivers team was CCed until today...

I can help with design, layout, and setting up the HTML pages, but someone will need to sort out the l10n/surveygizmo situation.

I'll work with Pascal on the L10N stuff. For the survey, we're going to drop the link for this version. Can you go ahead and set up the HTML page for this? Thanks

You're right, I meant to build the page but just realized that it's already available in Bug 615751 so I will go ahead and resolve this bug and pick up anything else needed for the page in Bug 615751. thanks
