Sounds like bug 547861

I cannot reproduce this with all href types, as bug 547861 states.
For build ID: Mozilla/5.0 (Windows NT 6.1; rv:2.0b8pre) Gecko/20101205 Firefox/4.0b8pre this is a WFM with these steps:

1. Navigate http://www.cnn.com
2. Browse the page down, until Top Stories pane is seen (left)
3. Left click on a story.
4. Ctrl + left click on a story

Behavior:

Step 3 opens story link in the same page.
Step 4 opens a new tab
Marking as a duplicated based on https://bugzilla.mozilla.org/show_bug.cgi?id=547861#c5
