I occasionally find that ctrl+t to open a new tab won't work unless I select the URL address bar (http://www.etc.com) first.  This is a persistant bug, coming from several versions of Firefox ago.

Reproducible: Sometimes

Steps to Reproduce:
1. Select another window
2. Select firefox
3. Ctrl-t to open a new tab.
4. If nothing happens, select inside the URL bar.
5. And then press ctrl-t.
Actual Results:  
Most of the time this comes from focussing the mouse on other windows (by selection), however it does appear to be somewhat random also.

Expected Results:  
Once selection of the URL bar has occurred, it's possible to use ctrl-t again to open a new tab.

This is a persistant bug, coming from several versions (major and minor) of Firefox ago.
