Firefox is rock-solid, so the only time I restart it is when I've installed an add-on. Twice, now, I've immediately clicked the "Tools" menu after a restart, only to see the SSD dialog appear as Firefox loads password-protected tabs. To replicate without a restart: hit "reload" on a page requiring authentication, before entering the master password. Quickly click the Tools menu before the SSD dialog appears. This action somehow "orphans" the onscreen Tools menu. I can't select things from the tool menu, and if I try to type my master password, the keystrokes go to the menu (selecting the menu items matching each typed letter). Escape and enter (for cancel/OK) still work. The only way I've found to escape this is to cancel the SSD dialog(s), at which point the Tools menu returns and I can make my selection.
