624038 - Add new field to mainstream Newsletter signup box on Mozilla.org Home Page

One or two changes to the existing Mainstream Newsletter signup box in the center of the page. 

a) Please change the subheadline to say
"Get our monthly newsletter" (currently says "Get our newsletter") 

b) Please help us add in an additional field for their email format preference. We need to add this across all our signup centers.  

Underneath the country drop down, we need to add two small radio buttons or check boxes for the email format. It can appear like this: 

o HTML   o Text

This should be right above privacy policy check box (exists already).  

The email format preference data needs to flow into the Responsys database into this field:  EMAIL_FORMAT_  (please note the second underscore)

To keep this sign up box in sync with the ones on mozilla.com I'd prefer to just copy from an instance where this change has already been made.  Can you please link to a bug about making this update on mozilla.com or to a commit where this change was made?
