628147 - add more info to socorro apache access logs

We should log more info in the access logs:

1) both incoming and outgoing size
2) timing info
3) user's IP, not just zeus'

There is already an extra log line for #3, (x-forwarded-for env=is-forwarded) but it does not seem to work in SJC or PHX (we just get the word "x-forwarded-for" on it's own line in the access log).

We should be able to get all of these on one line, I'll test in staging and get the log line we needed.

We don't want the user's IP, and the other stuff goes to syslog/statsd
