The problem is that (e.g.) entries from the history can fill up the limit of the autocomplete function (12 entries) and thus displace all entries that might match switch-to behavior, suggesting that no such tab exists.

To avoid such counter-intuitive behavior the limitation should apply per result type (history, switch-to, bookmarks, instant search [via addons], etc.)
