disable "Username:" text when username text field is disabled

Details
in the account manager, the ui for smtp servers, we disable the username text
field when the "always use username" checkbox is unchecked.
we should also be disabling the "Username:" text label.
fix in hand, sr=bienvenu. I'll check in today.

Reopening.
- After selecting the checkbox the Username: text and text field is still
disabled. I have to deselect the checkbox to enter text into the text field.
Then when I select the checkbox the text field is disabled.
I can't get into a state where it's checked and the Username text/text field are
enabled.

checkbox weirdness? regression probably a dup of that oncommand="" bug.
