in the account manager, the ui for smtp servers, we disable the username text field when the "always use username" checkbox is unchecked. we should also be disabling the "Username:" text label. fix in hand, sr=bienvenu. I'll check in today.

Build 2001-010804: NT4 Build 2001-010808: Linux 6.2, Mac 9.04 Reopening. - After selecting the checkbox the Username: text and text field is still disabled. I have to deselect the checkbox to enter text into the text field. Then when I select the checkbox the text field is disabled. I can't get into a state where it's checked and the Username text/text field are enabled.

checkbox weirdness? regression probably a dup of that oncommand="" bug.
