When you open a page in a new tab, this tab displays the correct icon. But when you changes to other page in this tab, the icon stays the same. It means, every tab shows the icon of the first page displayed;

Does the issue still occur if you start Firefox in Safe Mode? http://support.mozilla.com/en-US/kb/Safe+Mode

How about with a new, empty profile? http://support.mozilla.com/en-US/kb/Basic+Troubleshooting#Make_a_new_profile

The issue doesn't occur when you start Firefox in safe mode.

Problem seems to be in IE Tab add on. When it is disabled, the issue doesn't occur. If you enable it and restart Firefox, the issue returns.
