I have noticed with Firefox 4.0 that if you open the browser and customize the toolbar at the top, then save these settings.  If you try to customize the toolbar again it fails unless you restart Firefox first.  Also, settings are not being retained between restarts in all cases.  I have a couple of buttons that keep moving around on the toolbar.

Steps to Reproduce:
1. Open Firefox
2. Right click on the toolbar at the top and select "Customize".
3. Customize your layout.
4. Click "Done".
5. Right click on the toolbar at the top and select "Customize" again.
6. Now the toolbar will attempt to resize and show the menu bar if not visible, but the customize window will not appear.  Also, when restarting Firefox the settings are not saved and the buttons can be moved all over.  And some addons, like ForecastFox does not show up in the Customize box, but instead uses a different arrangement to place the buttons...
Actual Results:

Expected Results:  
The Customize window should open each time and the buttons should be saved in the location they are placed.

This is happening to me as well. And it's not always when I restart Firefox. Simply refreshing a page, or even opening a new one, results in the same. The load icon, Forecastfox, resizable spaces, and a few other random buttons move around on their own for seemingly no reason. This does NOT happen to buttons that are installed with Firefox. Only add-ons and extensions. 

It would be amazing if this were fixed because it's VERY annoying that things jump around out of custom arrangement and no matter how many times you put them back, they just keep going wherever they want to be.

I submitted bug 688111 with steps to reproduce an error similar to this!
Maybe that can be the cause of this.
