When someone triggers the "invalid reset code" error, show the original "forgot password" form that asks for a username, rather than the "enter new password" form which is no longer appropriate. Steps to reproduce: 1. Go to https://account.services.mozilla.com/forgot?username=floatingatoll&key=BLAH-BLAH-BLAH-BLAH 2. Enter some nonsense password twice and submit
