Mozilla/5.0 (Macintosh; Intel Mac OS X 10.6; rv:2.0) Gecko/20100101 Firefox/4.0 We've had multiple reports of this. Steps: 1. Install the KidZui K2 add-on https://addons.mozilla.org/en-US/firefox/addon/kidzui-k2/ 2. After restart, shut down Firefox again. 3. On restart, notice that the Add-on is disabled Note that it might not happen at step 2, i.e. more than one restart might be needed. The 'workaround' is to do 'Find Update' and compatibility kicks in again without actually an update occurring. Note also that the add-on was uploaded with a maxVer of 4.0b<something> but the compatibility to 4.0.* was set on AMO.
Are you seeing this yourself? Can you enable extensions.logging.enabled and see what gets logged each time? Can you get extensions.log files from people that are seeing this? This can happen if the database gets corrupt or something but that really shouldn't be happening much.
Works fine for me in Firefox 4.0 on OS X.

Not a lot we can do here without more info, please reopen if you can reproduce this again
