There's a problem with the numbering under "Please use this form to report any websites or mobile app stores that are:" There's a #3 that is next to the #2 entry, rather than below it, and then the numbering after that is off as a result. Here's how it looks:

1. Distributing harmful products under Mozilla brands such as Firefox.
2. Charging for Firefox, Thunderbird, or any of the Mozilla products. 3. Collecting personal information as a condition to download any of the Mozilla software (registration requirement).
3. Using the Mozilla trademarks to mislead the public into thinking the site is an official Mozilla site or sponsored by Mozilla...

Looks like this was fixed in trunk last month, but didn't make it into production. Marking as a DUPE of 630239 and will pick it up over there.
