It seems to occur when playing anything that includes full screen. The close, minimize, and maximize are the only parts that disappear.

It appears with Firefox 15.x and 16.0
It appears if firefox 22.0
i don't know if this regards with fullscreen flash videos, now i'm testing with default FF theme.
FF 23, bug repeats only now.
Black background at a button place.
