Details

STR: 1. Open firefox with a clean profile. 2. Go to AMO, and log in. 3. Dismiss the doorhanger notification by clicking somewhere in the content. 4. Click on it once, and dismiss it like step 3 again. 5. Go back one page in the history. The doorhanger icon disappears after step 5. It doesn't if you skip step 4, so step 4 seems to be a determining factor.
The notification persists for a certain amount of time, after which it is dismissed if you change location. So, I think what's happening is that doing step 4 is taking enough time to run down the timeout timer.
(Oops, submitted the comment before I meant to.) This was implemented in bug 595175. If you replace step 4 with waiting over 10 seconds, does the same thing happen?
