Build 2001011204.  This is the same problem as bug 64511.
Marking Duplicate as the reporter states.

Same problem, but not sure it's the same fix since it's another dialog.. I
reopen this one just in case.  Sorry for being unclear.

Confirming, sending to passsword manager (not sure this is where it goes).
Apparently it's also the bug about <textfield type="text"> producing a textfield
too small. The solution is either to fix the real bug (follow link in bug
64511), or remove type="text" from the dialog.

These dialogs should all be cleared up together using the work-around proposed 
in bug 51698.  That bug seems to be the master bug for all of these so I'll dup 
this one there.
