New with the release of FF4. A restart fixes the problem.

I'm using the Silvermel theme, but based on the problems which Pardel has had with tabs, suspect that it's the component, not the extension.

I don't think so; I've been using the default theme for a week, and not seen the problem, i.e., it appears to have something to do with the way Silvermel interacts with the tab component, but might have something to with add-ons (Pardel tells me that nobody else has reported the problem). Because the problem is intermittent, I'd rather not disable extensions to find out (but will if that's what it takes.

How often have you seen it before? Once, twice, or even more times a day depending on how often you restart Firefox. Sounds like its a high chance that Silvermel is the reason for it. Please switch between the Default theme and Silvermel just to verify it helps. Further please list all of your installed and enabled extensions.
Also which problems are you talking about in comment 0? Do you have a link?

Once or twice a week; since I start FF two or three times a day on average (the problem occurs whether or not FF is in my start-up directory), so it's (very) roughly every four or five times FF starts. As noted above, the problem (which is solved by restarting FF) does not appear when using the default theme. It's obviously a problem with the Silvermel-Tab module interface but, as also noted above, reading the comments on Pardel's blog raises questions as to which side of it.
Enabled Extensions: Adblock Plus, Pop-Up manager and Element Hiding Helper;
AVG Safe Search; Click&Clean; Fasterfox Lite; HTTPS Everywhere; IE View Lite; Lastpass; NoScript, Silver- & Cara-mel; Test Pilot and X-Marks.

Just happened again (on Start-Up). Yesterday, the tab close buttons were there but the window slider was missing. I'm a little concerned that Firefox is becoming unstable, but not sure how to report it.

If it only happens with the Silvermel-Tab module enabled you should get in contact with the theme author and point him to that issue. Can you please disable that theme and try with the default theme for a while? Is that reproducible in such a case?
