This has happened several times already. The latest example is this one:

See version 0.5.
What seems to happen is that the upload creates 2 duplicate versions, one of which is automatically admin-disabled. The other is submitted for review, but the file is inaccessible. Our only solution in this case is reject the add-on and ask the developer to upload again, which is very inconvenient for everyone.

This should be easier to debug now that more of our logs end up in the log files.
