Create an internal API to create an app receipt for an add-on, given a valid transaction that has not been refunded. 

See URL for spec.

If the add-on has been deleted then we should still be able to recreate the receipt, this might mean persisting the receipt in a model.
mhanson is also writing some code for this I think
