Today I have found one article...

URL looks like this article is translated but it is not. Problem is that there is revision which was rejected. So no approved revisions, no revision waiting for approvation etc.

I think such article should be marked as untranslated.
I see the yellow bar. What does it say? It should say this article has not been translated yet and tell you, that you can start translating it.
According to Google Translate, that's what it does say. As far as I can tell that is what is supposed to happen. Can you tell me what exactly you want to have changed? Is it the /cs/ in the URL that you don't want?

1) Visit https://support.mozilla.com/cs/localization/most-visited-translations
2) You will see article "Zásuvný modul Adobe Flash spadl" there. This article is marked as translated.
3) Click on detail. Article is not translated and you will see text that you should translate article.

Reason is described above.
Ah, okay, I see. The history showed a revision from November last month. Apparently that revision was created before we moved to the new system. Unfortunately there are some hickups with content from the old system that was imported into the new one. Since it had no real content I removed that article. It shows as not translated now.
