Are you sure you don't have two collections with the same name?

Could you check the URLs for the dupe collections? in Add to Collection box, click on "View this collection" link and check the URLs
I observed same issue when trying to create a new collection and it is possible that the collections were duplicated because the user pressed "Create collection" or "Add to collection" buttons many times. Also , the "Collection created! " message is displayed many times if the user repeatedly pressed "Create collection". Please let me know if a new bug should be logged for this issue.
