This should work much like the feature works on Github. So comments are related to specific versions of specific lines of specific files, and should be mutable. This would allow editors to add notes to code for future reference, or so that editors can chat amongst each other about code in a context that is useful.
This is already part of the overall plan to make a searchable code repo. You can see bug 457474 for more info. I'll leave this open though.
