Hey James, all--
Please add of a 'Date of Birth' drop-down menu on the Student Reps sign-up form: http://www.mozilla.org/en-US/newsletter/studentreps.
We're using the DOB field to prevent the form from storing data from minors and preventing those under 13 to sign-up for the program.
There's an additional component to this request. We'll need to have the form trigger cookies and scripts when minors enter there DOB. Please see the recent comments in bug 680308 for the details for these components, and let me know if I need to open additional bugs.
Student Reps is launching next week (19 Dec 2011), so adding this feature is requested ASAP.
Please let me know if you need more details/discussion.

This is a really last minute change that's more than just a tweak. We need more time for bugs like this to get in to the queue if it has to be done by the next business day. We'll see what we can do but we really need more notice for changes like this.

Thanks for the chat and deeper insight into the workflow, James.
Jishnu and Liz offered further feedback following our discussion, which has changed the request. We no longer need to add the Date of Birth field or the other previously mentioned components. Instead, only need to focus on folks in high school. So we need to:
- Delete the 'Other' field next to the Level of Study string
- Create another landing page where anyone who submits the form with 'Secondary/High School' in the Level of Study menu will be directed. On that page, we'll have the parental consent form for minors available for download and directions for printing and returning the form to us.
- Disable the sign-up form from archiving data from those who submit the form with 'Secondary/High School'
I'll finalize the content with Jishnu and post the bug Monday.
Do we need to request resources from the Creative Team for the landing page?

Hi, Liz, Jisnhu--
Before we engage WebDev resources, it looks like we'll need to either create a new parental consent/minors registration form for the Student Reps program or re-purpose the one used for the Mozilla Reps program. Reason being: Student Reps are not held to the same standards as Mozilla Reps. (Any student can be a Student Rep.)
Early this week, my team will sign-off on the Student Rep roles, responsibilities, and project areas, which will be available on the wiki: https://wiki.mozilla.org/ReMo/Student_Reps. I'll update you here when it's ready for review.

Hi Jason,
If it's a different workflow, I think we don't need a web form because we'd have to get parents to sign up or authorize their accounts so a written form along with the consent would work the best, right?

Jason - did you mean we need to have a PDF of a parental consent form specifically for Student Reps under 18 to post on the Student Reps sign-up site for download as described in comment 4? Do you mind filing a legal bug requesting that?

Hey James,
Per this Privacy/EULA bug -- https://bugzilla.mozilla.org/show_bug.cgi?id=712408#c15 -- we need to make the following changes to the Student Reps sign-up form as soon as possible:
Source page: http://www.mozilla.org/en-US/newsletter/studentreps
- under the home email field, adding the following text in a small font (similar to the privacy statement at the bottom): 'You must be 18 years of age or older to join the Student Reps program.'
- remove secondary/high school from the 'Level of Study' drop down menu

Hey James,
A few more changes to the sign-up form as follows:
- Change first line to 'Show your classmates why Firefox rocks, and encourage others to contribute to the Mozilla project.'
- Create a heading entitled 'Student Reps Registration Form' using the same font as the text above; insert under 'Be a Student Rep...' leaving space between the two lines.
- Insert 'You must be 18 years...' directly below 'Student Reps Registration Form' heading
- Leave space and then continue form with 'Personal Info'
- Under 'Home Email' field, add in small font 'If you already registered as a Student Rep, you can use this form to update your record with us. Just use the same email address you used before.'

I have a non-legal comment. I think the smallest font is too small. It's barely legible.

I agree with Liz's comments as do a few other colleagues who've seen the form.
Next steps:
- increase that small font by one (1) point wherever it appears throughout the form
- the following line should appear as it does in the current form (http://mzl.la/stu-reps-signup): "Show your classmates why Firefox rocks, and encourage others to contribute to the Mozilla project."
