Most important the logout link was missing. Screenshot attached below as FF9b6-5.png showing the difference. Expected results: See screenshot FF9b6-5.png
I just checked this again with FF8b5 and it is also screwed up. It does work fine with Chrome. I then checked it with FF8 and it is screwed up. It appears that the website has done something that renders incorrectly in FF. The change had to have occurred within the last couple of days.
Just talked to mymedicare.gov technical support. They told me that the site "sometimes" works with FF and "sometimes" doesn't. They told me to use Safari since I'm on a Mac. I don't know if Mozilla has a way to "poke" them; if you do please POKE them.
Problem solved. I tried with a blank profile; all OK. I traced the issue to Adblock Plus. I've noticed on Mozillazine a rash of issues going back to ABP lately. Sorry for the false alarm.
