Found in bug 715290.
An addon's directory's file modified timestamp was being changed. The addon was incompatible according to the install.rdf (but compatible according to AMO). So it was disabled on restart. However, there was also a theme enabled, which got disabled. When XPIProvider disables the addon, it blindly enables the default theme without checking that the addon is a theme or not.
