720039 - Remove getsatisfaction support

I ran some numbers and the number of people using getsatisfaction is very low.  This bug is to remove the special getsatisfaction support from AMO which will free us up from another JS request and some special case code.  This also shrinks our whitelist for CSP.

What do you have in mind?  People are still welcome to use getsatisfaction URLs, there just won't be a special popup about them on AMO.
