It had some rpm database corruption, corrected using http://www.rpm.org/wiki/Docs/RpmRecovery, then puppet complains that /usr/bin/env is missing and it won't install mercurial. That env is a symlink to a valid file, just like other slaves, so I'm thinking file system is not in a good way. Lets try reimaging.
