Filing as UNCO because details are sketchy. From the first day of Firefox 10 uptake:

https://support.mozilla.org/en-US/questions/916542:

deleted extensions on update

Firefox automatically updated then all the extentions vanished. I started downloading the extensions again but whenever I press on the new Extensions Options, the browser freezes and has to be shut down by Task Manager. Is there a way to get the original extensions and settings back like they were before the update?

https://support.mozilla.org/en-US/questions/916464:

why are extensions hidden in firefox 10?

no extensions showing in firefox 10

We could do with getting copies of extensions.sqlite, extensions.ini and extensions.log from the profile of users seeing this, unfortunately the two support requests have already been told to delete those files.

That sounds suspiciously like a corrupt DB.
My guess is that this might be bug 671894
I've asked Blair to take a look at bug 671894 once he's back from PTO.
So a few comments: it sounds like it fixes itself for some people. Maybe they tried to install an extension or something? No idea.

Any update on this bug? This is not locally reproducible. Any further details on this one, Cheng?

I'm having the same issue with Fx 17 beta (I don't know if it happened during upgrade of Beta 5 -> Beta 6 or during "normal" browser usage). I do have some logs that might interest you (Fx 17 Beta 6 on Mac OS X 10.8.2 on MBA 2012, several web dev addons):

extensions.log:
###############
2012-11-20 01:09:15 ERROR addons.xpi-utils: Failed to open database (1st attempt): [Exception... Mac OS X kernel log (when shutting down Fx):

Don't know if this is related but seemed interesting.

I will restart my system and check if this solves my problem, otherwise I'll delete the extensions.sqlite and let it recreate. (BTW, my extensions.ini looks ok.)
Oh, I do have the Keychain integration addon installed, so this might also be the cause for the DYLD warning.
Sorry for bugspam. I had bug 671894 open in another tab but didn't read it yet.

This issue might have been solved by Bug 671894. Will wait for the upgrade later today.
Yea, AFAICT, this is another case of bug 671894, which is fixed in Fx18.
