Details

User Agent: Mozilla/5.0 (Windows NT 6.1; rv:13.0) Gecko/20120306 Firefox/13.0a1
Build ID: 20120306031101

Steps to reproduce:

Attempted to save a file and got as far as Clicking on New Folder.

Actual results:

A dialog box:
Microsoft Visual C++ Debug Library
Debug Assertion Failed!

Program: C:\Mozilla\Firefox\firefox.exe
File:
f:\dd\vctools\vclibs\ship\atlmfc\src\atl\atls\atldebugapi.cpp
Line:50

Expression: pModule != 0
For information on how your program can cause an assertion
failure, see the Visual C++ documentation on asserts.
(Press Retry to debug the application)
...appeared and then Firefox went all not responding.

Expected results:

A new Folder should have been created and Firefox should have continued responding. Failing that, more details on the assert should have been given and clicking on the dialog box buttons should have been possible.

Does the issue still occur if you start Firefox in Safe Mode? http://support.mozilla.com/en-US/kb/Safe+Mode

How about with a new, empty profile? http://support.mozilla.com/en-US/kb/Basic%20Troubleshooting#w_8-make-a-new-profile
The issue occurs when Firefox is very low on memory.
Is this still reproducible?
