various places throughout the code use NS_NewStreamLoader to load an entire
document in memory.  this is, for example, used for JS and CSS files referenced
from HTML.  (perhaps these should really be asynchronous?)

at any rate, i noticed that not all instances of NS_NewStreamLoader pass
proper notificationCallbacks.  there also seems to be problems handling
reload and shift-reload correctly.

the reload and shift-reload problems were fixed along with my patch for bug 75679.
