App job that creates new versions if manifest has changed isn't working

Manifest changes are supposed to result in new versions being created, due to a cron job that runs daily and creates a new version if the manifest has changed.

This isn't happening right now - manifests are updated a number of times, over a multiple days, but there isn't a new version created.  (The only multiple versions I can recall seeing were due to us changing the manifest url)

See url for an example app that was rejected 3 times for issues in the manifest but only has a single version.
