When you are off the origin of an app, show the name of the app in the title first, then the off-origin URL

Nit-pick on bug 771395 for the title of the UI - when you are off the origin of an app, I'd suggest showing the name of the app first, then the off-origin URL. We also might want to be possibly a bit more explicit at some point that "hey, you aren't in the app's context" somewhere, but that might be a separate bug (it's evident in the title, but I'm not sure it's obvious to a user). Title suggested: <app name> - <off-origin URL>

I'm not sure about the reasoning behind the suggestion, but the reason we put the origin into the title bar, which was suggested by security reviewer dveditz (cc:ed), is to warn the user that they're on a different origin. And I'm concerned that putting that app name in the title will make this warning less noticeable, especially if the app has a long name. Thus I recommend that we don't do this, although I do like the second suggestion to be more explicit about being outside the app's context.
