Exclude already-open pages (or maybe sites) from New Tab page

The New Tab page is great for navigating to pages I access often or I accessed recently. This is watered down a little by the fact that these exact pages are usually already open in tabs of their own when I open a New Tab page. These have a long lifetime, and opening and closing tabs happens much more often with pages of prime-but-not-quite-top-level importance -- often just over the edge so that they're not on the New Tab page any more. Thus, it would be very useful if already-open pages (which I wouldn't want to select anyway) would be left out from the list of pages considered for the New Tab page. This would make room for pages with nearly as high a usage rank, but a much much higher probability of being useful to me.

[For bonus points, pages removed from the list because they're already open in a tab of their own could be displayed in a strip on the left-hand edge of New Tab page or something, allowing me to switch to the respective tabs.]
It would also be good to filter home pages from this list as they are opened automatically, often, and tend to dominate the top sites list.
Hypothetical fix seems similar to that for #710392.
