776298 - Change GetAppManifestURL to GetAppManifestId and create content processes for app per id

GetAppManifestId() would be more useful than GetAppManifestURL() generally speaking.
In addition, when we create a content process, instead of having a hashtable associated manifest URLs to content processes, we could associate IDs to content processes. The only issue I see whit doing that is that an ID, while the phone is running, can be assigned to different apps. However, with bug 775327, that should not happen.

That really has nothing to do with Apps but with nsFrameLoader and BrowserElement.
