Recreation of duplicate most visited link on bookmarks toolbar on each session start has existed too long to remember, at least prior to 2.12b1. I can delete either or both while running, but always two appear on startup.
Profile is old, contains 3 files last written in 2007, used currently only for 32 bit release-next on Linux, but originally for trunk when it was called trunk. Many times it has had user_pref("browser.places.importBookmarksHTML", true); in user.js, for a bookmarks.html of about 500k exported from SM 2.7.x and prior on OS/2, though not in recent weeks. 3 extensions: Domi, CZ & Preserve Download Modification Timestamp.

This went away in 2.13b6.
