Saw the problem on
Firefox
Version: 19.0a1
User Agent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10.8; rv:19.0) Gecko/19.0 Firefox/19.0
Build ID: 20121008170935
while running the MozTrap test case #2719 -> Oct. 9-15,2012 Sync Community test run of services-central.
prerequisites:
1. Sync Account is setup and 2 clients are are connected to it. We'll call them client A and client B
2. those two clients have at least one add-on synced
note: add-on sync support is currently for AMO supplied add-ons only and for desktop to desktop only.
STR:
1) On client A, go to Add-ons Manager. uninstall one of the add-ons there. Sync Now via the menu item on client A
2) go to client B and Sync Now via the menu item
Actual Results:
The addon is disabled on Client B and not removed.
Expected Results:
The uninstalled add-on on client A should also be uninstalled in the Add-ons Manager of client B. (restart may be required to make it actually take)

Federico, does as restart of Client B uninstall the add-on in question?

No, I tried. Restarting Client B doesn't help.

Adblock Plus 2.1.2

Interesting, that is a restartless add-on. Please try with an add-on requiring a restart to install/uninstall.

I believe I figured this out. Even though adblock is restartless, the removed state doesn't get synced until the browser has been restarted. With standard addons, the removed state is also not synced unless the browser has actually been restarted, then synced.
The correct test case steps should be:
1) Remove addon from client A
2) Restart Client A
3) Sync Now Client A
4) Sync Now Client B
5) Restart Client B
