Details
User Agent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10.6; rv:16.0) Gecko/20100101 Firefox/16.0
Build ID: 20121024073032
Steps to reproduce:
RE: WinXP SP3 running on an AMD system
1st, installed update to 16.0.2 in normal way.
On 2nd try, performed a complete uninstall, and a clean install.
Actual results:
In both cases firefox.exe appears as 2 tasks in taskman.
But FF window never opens.
[Taskman can end one of these processes, but not both of them]
Expected results:
Add-ons should have been checked for compatibility, and FF window should have opened.
Status is that FF is not available for work on target system.

Have no FF window, regardless that taskman shows a FF task loaded.
So about the only thing to do is to try a safe start with the shift key.
Now taskman shows 2 tasks firefox.exe--same as I initially reported.
But still no FF window, no error message.
Neither task is showing any cpu usage.

Can you start Firefox in Safe Mode? (Safe Mode disables extensions and themes, hardware acceleration and some JavaScript stuff in order to exclude some possible reasons for problems. It does not disable plugins which are add-ons.) See http://support.mozilla.com/en-US/kb/Safe+Mode
Can you start Firefox with a new and empty profile? See http://support.mozilla.com/en-US/kb/Basic%20Troubleshooting#w_8-make-a-new-profile and http://support.mozilla.org/kb/Managing%20profiles .

Not an application update bug since the steps in comment #0 used the installer. This is likely due to either add-ons or possibly something unusual about the system itself so moving to Firefox General for further triage.
