Mozilla should init PSM/NSS once (at startup?) and keep a reference to it. 
During the lifetime of the browser the ref count to PSM should not go to zero 
unless the browser is exiting. PSM/NSS should be initialized once because NSS 
maintains state (entropy, authentication info for the profile etc) that is lost 
each time NSS is initialized.
