I'm not sure what other steps to take to troubleshoot
escalated to justdave in IRC. Might be a transient issues cause by puppet.

:justdave found and fixed the puppet issue, which was that $secrets::nagiosdaemon::asterisk::username was undefined. That variable is now defined and monitoring has been confirmed OK, as of sysadmins r55472.
