steps to reproduce:
1. Load https://marketplace-dev.allizom.org/developers/app/test-app-poea9842/payments/
2. Click on 'Add account' and set up new bank account
3. Save changes
observed behavior:
Bank account creation fails with a 500. The error message is nested within the account creation lightbox and has overlap issues. See screenshot.
