Details
I have been maintaining the IRIX port of Mozilla for the last 5 months for SGI,
using zeroJ@null.net and more recently johnv@adacel.com.au
Richard Hess has agreed to be a voucher if that is applicable, otherwise Chris
Seawood and Wan-Teh Chang are probably the most familar with my bugs/patches.

In contact information, are the Phone Number and Address to be my personal
details or those of the company I work for?

Richard, can you please have a look at the form, and inform me of any values
SGI would like to specify, e.g. email address I use to commit with, what I
should put in 'Employer', and whether the Contribution Form terms are all
within our SiSE Agreement.

I have faxed the form, and I will be sending the original shortly, however I am
not sure of the timeframe it will take to arrive at Netscape from Australia.
If it is needed soon, I will send it express.

Form received. John - no need to express mail the form, just send it regular
mail.

Marcia, I have recently resigned from Adacel, and as a result I will no longer
be contributing on behalf of SGI, and the form you have is no longer valid.
Sorry for any inconvenience caused.
