As per http://www.w3.org/TR/xml-stylesheet/ ("Associating Style Sheets with XML
documents"), 

"The xml-stylesheet processing instruction is allowed only in the prolog of an
XML document. The syntax of XML constrains where processing instructions are
allowed in the prolog; the xml-stylesheet processing instruction is allowed
anywhere in the prolog that meets these constraints."

Currently the PI is recognized even when it is inside the document element.

(This started out as bug 85161. As this is a standards compatibility issue, and
the original trouble just vanished without explicitly being fixed, I'm filing
this one separately.)
