I sometimes get this one when doing some rightclicking (cut, copy and paste,
find) in the personal toolbar.

to reproduce everytime:
1) press Manage Bookmarks
2) press File Bookmark(s)
watch the console...:)
OS: Windows 2000 → All
Hardware: PC → All
Hmm, I don't see this one, but I do see this new one:
