Can you reproduce this without any add-ons enabled (try using Help > Restart with Add-ons Disabled) ?

Attached the screencast with additional info. In short, it is working as expected when it restarts in safe mode (with extension disabled). However, If I manually disable all the extension, I was able to reproduce the issue. Additionaly it happens only at my work machine, I was not able to reproduce it with machine at home. The set of add-ons & apps are mostly same on both machine. I even try to identify the extension by disabling and enabling them one by one without any luck. I guess it is a edge case, I will post more info as soon as I zero in on exact reason. Thanks for looking into the issue. I was able to isolate the issue. It is due to extension DownThemAll 2.0.16, I was able to reproduce the issue even if DownThemAll extension is disabled. I believe it is modifying part of Save dialog which cause the issue. Remove/ Uninstalling the DownThemAll extension solves the issue.
