Details
User Agent: Mozilla/5.0 (Windows NT 6.1; rv:21.0) Gecko/20100101 Firefox/21.0 (Beta/Release)
Build ID: 20130511120803
Steps to reproduce:
Selected to shutdown via the 'caption buttons' at top right.
Actual results:
UI closed, but the application would not close properly. Will close when using alt+f4.
The information screen says "application shut down unexpectedly" other than normal.
Extensions are adblock plus, add bookmark here, automatic save folder 1.5, bulk image downloader, download manager tweak, download youtube videos as MP4, Roboform, Tab Mix Plus.
Expected results:
The application should shut down without any improper dialogs.

I might have found the problem. Keep up the tracing anyway. The extension DownloadHelper, did not remove completely when first tasked, causing the browser to load improperly - and may cause this error. However I continue to use the alt+F4 combination on the browser close, just to be sure.

This means it's an addon issue and not a Firefox one. You should send a notice to the addon's developer so he can fix this.
