As a note, please make sure to *always* unplug a monitor from the iX boxes after you're done troubleshooting. We can't put them back into production (they will have the wrong resolution) if a monitor is left plugged in.

:arr, what's the production resolution here? 1600x1200? If we began using monitors with the same native resolution, would it help during the troubleshooting/diag process?

My understanding is that not only does it screw up the resolution, but that the bigger problem is that it creates a *third* monitor (onboard, the virtual one for the tests, and the real plugged in on) which breaks things. We just happen to catch it fairly easily because the resolution isn't right. I'll let Q be the definitive answer there, though.

There was no monitor plugged into this host, I rebooted the host.
was there an extron or adapter plugged in? The system was reading an edid.
aand it kept the same id through several reboota and driver/device removals and readds

Nope. I did pull the system from the chassis for a few seconds since that fixes most weird issues (ipmi).
