Communication can get messy if we tie all the notes to a single parent thread.

Consider a situation when a reviewer requests for more info, a new note will be created in the thread. The thread has a single context now. But lets say, the reviewer rejects the version/app that the thread is based on. A note saying that the reviewer has rejected the app is tied to the same thread. Now the thread has two different contexts. When someone posts a new note (as a message) its hard to determine what context the note belongs to.
