895571 - Check donation form missing and should be restored

On the legacy mozilla.org site there was a pdf file that was hosted that could be used by people who wanted to send in a donation by check.  That was at:

https://www.mozilla.org/foundation/donate_form.pdf

I just checked the link today because someone was interested in donating by a check and I'm getting a 404 error.

The link is being linked to still by live pages, including:

https://join.mozilla.org/faq.en.html

We should restore the link because there are people who want to donate to Mozilla by check.

The old file can be found at

It looks like the file needs to be added to

http://viewvc.svn.mozilla.org/vc/projects/static.mozilla.com/trunk/foundation/documents/

and then a redirect added to bedrock, probably around here:

https://github.com/mozilla/bedrock/blob/master/etc/httpd/global.conf#L365
